The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or third parties will be dealt with in accordance with the Act. If you wish to request access to the records held by the practice, please contact our reception staff on 0121 475 7997.

From 01 April 2016 patients will be able to request online access to view selected sections of their medical records, such as test results. Please contact our reception staff regarding this.  Requests for this service are subject to the provision of suitable ID and are granted at the sole discretion of a GP.  A GP may refuse permission if they consider it would not be in a patient’s best interests to view their records.
