Shenley Green Surgery welcomes new patients.

You can register by completing a registration form (GSM1) which can be downloaded here.  When you register, we may ask you to make an appointment to see the practice nurse (or doctor if you are on regular medication) for a new patient health check.  This will ensure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.

If you are housebound please call the Surgery for details on how to register without attending the surgery.  We always require some form of confirmation of your house address at the time of registrations e.g. utility bill and or bank statement, registration of new baby will require birth certificate at the time of registration.

If you are born outside the UK or are coming from abroad, we will also require confirmation of the eligibility of NHS entitlement e.g. Valid Passport and or visa; full-time student status, letter of the contract of employment etc.

All New patients over the age of 5 years registering with the practice will require having a new patient health check with the Practice Nurse and or Health Care Assistant prior to seeing the doctor as it can take some time for your records to be transferred from you previous doctor.

We can only accept new patients who live in our practice catchment area.  Our catchment area is the geographical area we cover as a Practice.  If you live within this area you should be eligible to register with us.
