Sick Note/ Med3 Requests

How long have you been ill for (inc bank holidays) ?

‘I have been ill for 28 days or less.’

You can complete a Self Certification form yourself. However some employers insist on a Doctor’s note regardless. A Doctor’s note under these circumstances may involve a charge. Some employers will have their own self-cert forms. However if they do not, you can download the Self Cert Form. Please print it, fill it in and hand it in to your employer. You do not need to see a Doctor. Please click on the link below:

‘I have been ill for more than 28 days’

If your illness is in relation to COVID-19. You will need to self-isolate and following the current government guidelines. You may self-validate for seven days.

After the seven days, you can request an email confirmation of the diagnosis by dialing 111, avoiding an unnecessary trips to your Doctors or hospital.

If your illness is not COVD-19 related.

If you have already had a Sick Note (Med3) for this illness or you have recently been reviewed by one of our Doctors regarding the illness the doctor may not need to see you to issue an another Sick Note. Call the surgery on: 01214757997  and select option 1.

Please note that you may be asked to be booked in for a review discuss the request. Otherwise, if your request is approved will we send your sick note to you by SMS, email or post.  We are unable to issue a sick note before the date it runs out. So you your sick not will  be generated on that day and not before. Please DO NOT call before the date that your sick note ends, as you will not receive a status update from reception or the administrative team.
